Family Psychosocial Screen This screen, developed by Kathi J. Kemper, MD, MPH and Kelly Kelleher, MD, MPH, screens family health habits, maternal depression, substance use, parent's history of abuse, etc.
Pediatric Symptom Checklist Developed by Michael Jellinek, MD, this 35 item parent questionnaire screens for both internalizing and externalizing problems, as well as attentional problems.
Safety Word Inventory and Literacy Screener (SWILS) A 29-item measure of childrens academic skills that can also be used as a springboard to injury prevention counseling. Normed on 934 children around the country, the SWILS is 78% sensitive and 84% specific to academic status in the areas of reading, math and written language. For children from 6 to 14 years of age.
Modified Checklist of Autism in Toddlers This screen may be downloaded here along with a second file on how it is scored. The MCHAT is online together with PEDS at www.forepath.org. Additional information about using the MCHAT to comply with recommendations from the American Academy of Neurology are detailed at www.firstsigns.org
Scored PEDS test forms Download scored examples of the three PEDS forms here. These may not be used to screen other children but can be shared with colleagues for training and review purposes.
